How Proactive Development are using Log my Care to strengthen their proactive care approach

Proactive Development supports 56 adults with learning disabilities and complex needs across supported living and residential services, with a team of 240 staff. Delivering highly personalised, 1:1 and 2:1 care, their approach depends on consistency, visibility, and a deep understanding of each individual they support.

As the organisation has grown, so too has the challenge of maintaining that level of oversight across multiple services. For Proactive Development, Log my Care has become a core part of how they meet that challenge, helping them move from reactive care delivery to a more proactive approach that is led by data and insight.

Real-time oversight across every service

For a leadership team responsible for multiple sites, visibility is essential for the smooth running of daily care operations.

Before Log my Care, understanding what was happening across services relied on a mix of systems, phone calls, and paper records. Information was often delayed, and gaining a full picture took time.

Now, everything is live, centralised, and accessible from anywhere.

This has fundamentally changed how the team operates. Oversight is no longer something that happens after the fact. Instead, it is continuous and available to the team no matter their location.

Staying ahead of incidents, not reacting to them

One of the biggest shifts has been in how incidents are managed.

Instead of reviewing incidents retrospectively, the team can now see them as they happen, alongside the wider context around that individual.

This has made it much easier to spot patterns and step in earlier.

For example, by looking at behaviour alongside seizure data, the team can begin to anticipate when an individual may be approaching an episode. Staff can prepare, adjust support, and reduce risk.

It’s a clear move away from reacting to events, towards preventing them.

Using evidence to involve families and reduce unnecessary medication for an individual

For Proactive Development, one of the most powerful uses of Log my Care has been in supporting better, more informed decisions around care.

In one case they were able to make meaningful and proactive changes to medication for an individual in their support. This individual had come from a previous service where they had experienced poor treatment and believed they had been pushed down the stairs. As a result, they began going up and down the stairs on their bottom. This behaviour was assumed to be caused by tremors, and they were prescribed medication to manage it.

After moving to Proactive Development and building trust with the team, the individual began walking up and down the stairs independently. The team felt the medication was no longer necessary but needed agreement from the family before making any changes.

When the family visited, the individual reverted to their previous behaviour, making it difficult to demonstrate the progress that had been made.

Using Log my Care, the team were able to share photos and videos through the family portal, clearly evidencing that the individual was walking up and down the stairs day to day. This gave the family the confidence to agree to a review of the medication, which was subsequently stopped.

Since then, the individual’s quality of life has continued to improve, and they have gone on to come off a further four unnecessary medications.

This is a clear example of how better visibility and shared evidence can support more collaborative decision-making — and ultimately lead to better outcomes for the people being supported.

Smooth adoption and a system staff actually use

Staff record care as it happens, rather than trying to remember and write it up later. That means records are more accurate, more detailed, and more reflective of real life.

It also opens things up for staff who may have found traditional documentation difficult. Logging becomes part of the flow of care, not something separate from it.

This has led to greater outcomes for the people in their care. More goals are being achieved, and more time is being spent on activities that they enjoy.
