We’ve launched AI-powered Care Plan Audits, a new way for care providers to review care planning documentation across their entire service in minutes. By helping teams spot gaps, inconsistencies and areas for improvement that might otherwise be missed, it gives them better information to act on and more confidence that people are receiving the right support.

From manual sampling to the complete picture
Auditing care plans thoroughly takes time. For busy teams, that can mean reviewing a sample of documentation rather than getting a complete picture across every person they support.
Care Plan Audits changes that by using AI to review every person’s active care plan alongside their profile, risk assessments, goals and consent forms, as well as incidents from the last 30 days.
Instead of relying on spot checks, teams can see a much more complete picture of care planning quality across their service and quickly understand where improvements may be needed. AI can do the heavy lifting of reviewing large amounts of information, while care teams bring the knowledge, experience and judgement to decide what to do with it.
Understanding the results
Each person receives a quality score out of 100 across six areas - depth, risk, consistency, grammar, specificity and independence - making it easy to see where documentation is strong and where attention is needed.
But a score on its own only tells you that something needs attention. Care Plan Audits also surfaces specific findings and takes you directly to the relevant care plan, risk assessment or goal, so you can understand what needs attention and why without searching through records to find it.
Once changes have been made, you can rerun the audit and see how the documentation has improved.

As we introduce more AI into Log my Care, we want to be clear about the role we believe it should play.
Care Plan Audits views the quality of documentation, not the quality of care or the people delivering it. Think of it as another pair of eyes, helping your team see where to focus while keeping them firmly in control.
The AI never changes or writes to care records. Every decision about someone’s care remains with the people who know and support them. The technology helps surface what matters; people decide what to do with that information.
We’ve taken the same approach to data and privacy. Customer data stays within Log my Care’s existing UK hosting, remains compliant with UK GDPR and is never used to train AI models.
These principles are fundamental to how we’re approaching AI at Log my Care: building technology that care teams can genuinely use and trust, with people always at the centre.
Why this matters
When information sits across care plans, risk assessments, goals and other records, it can be difficult to spot where something is missing or no longer consistent. Reviewing more of that information together can help bring those gaps to the surface sooner, giving care teams better information to make decisions and act.
